Starting January 1, 2026, our office will implement a $50 fee for all forms and letters requested for provider completion. This includes (but is not limited to) disability forms, FMLA paperwork, work or school notes requiring detailed medical information, insurance forms, and any other documents that require provider review and signature.
This fee helps support the time and administrative resources required to complete these documents thoroughly and accurately.
